Multiple Teams · Time to “Take Care of Business” with Physicals & Concussion Statement

With the start of the fall sports season just two weeks away, Olivet High School student-athletes are urged to “take care of business” with regard to their required paper work.

Under Michigan High School Athletic Association rules, all OHS student-athletes must pass a physical examination after April 15 of the previous school year and submit the completed form (signed by a physician and parent/guardian) to either their coach or the athletic department. Forms for athletic physicals can be obtained under “Eagles HQ” on the olivetathletics.com Web site or in the OHS main office.

In addition to a physical exam, student-athletes and their parents also must adhere to the new Michigan Sports Concussion Law and submit a signed statement acknowledging receipt of general concussion information and protocol. This form can be obtained under “Eagles HQ” or on the Michigan Department of Community Health Web site: http://michigan.gov/documents/mdch/Parentandathleteinfosheet_415328_7.pdf
For more information, contact OHS athletic director Matt Seidl at 269-749-3671 (ext. 3312) or 734-358-6136.
